Critical appraisal:

A CASE OF A 45 YEAR OLD MALE WITH ACUTE KIDNEY INJURY ON CHRONIC KIDNEY DISEASE (HYPERTENSIVE NEPHROPATHY) WITH URAEMIC ENCEPHALOPATHY.

Chief compliants are

  • Fever--10 days back and lasted for 3 days.
  • Lethargy.
  • Unable to concentrate (altered sensorium).
  • Pedel edema with whole body swelling (Anasarca).
  • Shortness of breath at rest.
History of present illness:

- No H/o fluid loss
- No H/o decreased urine output
-No H/o drug intake

History of past illness:

- Hypertensive for 5 yrs and used T.STAMLOBETA OD
- Chronic kidney disease for 5yrs and treated.
- No DM,CAD,ASTHMA & TB.

The case presentation i.e.,the blog of this case is simple and understandable.

History taking is clear and personal history is also adequately provided which is very important in provisional diagnosis.

Overall well covered case.

Diagnosis: 

Acute kidney injury on CKD,

HYPERTENSIVE NEPHROPATHY with uraemic encephalopathy.

Problems :

  • Pedal edema 
  • Anasarca( i.e.,General swelling of whole body)
  • Altered sensorium( unble to concentrate)
  • Lethargy
  • Shortness of breath at rest 
From past history

  • Hypertension
  • Chronic kidney disease
  • Pallor is present

They performed ECG as the patient is hypertensive in past.

CBP as the patient presents pallor.

Complete urine Examination- as patient has chronic kidney disease in past.

Reflect and share your telemedical learning experiences from the hospital as well as the community patients over the last month especially when you were E-logging 

My opinion on telemedical learning and Elogging:

Telemedical learning is helping us in a better way...
Initially I felt bad by thinking that I'm going to miss clinical postings in my 3rd sem due to the pandemic situation......But this way of learning that is telemedical learning and Elogging felt interesting.

Going through elogs that are made by my seniors  provided a widespread exposure to variety of cases in a short span of time.

While going through all the blogs that are prepared by my seniors I learnt a lot and it will be useful for me in future for sure.

Elog Idea is much helpful for us , as it is encouraging us to learn more subject. 

We are learning how to approach a patient,about History taking and also how to present the case.

Overall  It is a great experience,gave scope for interacting and we are learning a lot from this postings.
